Survey Templates

Survey templates are reusable survey designs that define the questions, layout, branding, and signature requirements for a survey. When you create a campaign, you select a template as its foundation. This ensures consistency across campaigns and simplifies setup for recurring events.

Accessing Survey Templates​

Navigate to Settings > Survey Templates in the left sidebar. The template list page displays all existing templates with the following information:

Created ByThe user who originally created the template.

Creating a New Template​

  1. Click + New Template at the top of the template list page.
  2. Fill in the template details:
    • Template Name -- A descriptive name for internal reference (e.g., "Q1 2026 Availability Survey").
    • Survey Type -- Select one of the four survey types: Availability, Confirmation, Travel, or Exit.
    • Description -- Optional internal description explaining the template's purpose or intended audience.
  3. Click Create to open the template editor.

The Template Editor​

The template editor is a drag-and-drop interface for building your survey. It is divided into three main areas:

Adding Questions​

To add a question to your template:

  1. Locate the desired question type in the Question Palette.
  2. Drag it onto the Canvas at the desired position.
  3. Click the newly added question to select it and open its properties in the Properties Panel.
  4. Configure the question text, options, and settings (see Question Types below).

Reordering Questions​

Drag and drop questions on the canvas to change their order. The order in the editor is the order caregivers will see when completing the survey.

Deleting Questions​

Select a question on the canvas, then click the trash icon in the Properties Panel or press the Delete key. You will be prompted to confirm the deletion.

caution

Deleting a question from a template does not affect surveys that have already been sent using this template. However, future campaigns created from this template will not include the deleted question.

Question Types​

The platform supports six question types. Each type is suited to different kinds of information collection.

Single Choice​

Presents the caregiver with a list of options, of which they may select exactly one.

Use cases: Yes/No questions, availability confirmation, single-selection preferences.

Multiple Choice​

Presents the caregiver with a list of options, of which they may select one or more.

Use cases: Preferred shift types, available dates from a list, certifications held.

Text​

A single-line text input field for short answers.

PlaceholderOptional placeholder text shown inside the input before the caregiver types.
RequiredWhether the caregiver must provide an answer.
Max LengthOptional maximum character count.
ValidationOptional input validation (e.g., email format, phone number format).

Use cases: Name fields, email addresses, phone numbers, short responses.

Long Text​

A multi-line text area for extended responses.

Max LengthOptional maximum character count.
RowsThe default height of the text area in rows (e.g., 4 rows).

Use cases: Special requests, detailed feedback, additional notes, travel accommodations.

Date​

A date picker that allows the caregiver to select a calendar date.

Use cases: Preferred travel dates, earliest available date, license expiration date.

Signature​

An electronic signature capture field. The caregiver draws or types their signature on a digital pad.

Question TextThe text displayed above the signature field, typically a legal acknowledgment or terms statement. Supports rich text formatting.
RequiredWhether the signature is mandatory for submission. Always set to required for Confirmation surveys.
Legal TextOptional fine-print text displayed below the signature pad (e.g., "By signing above, you agree to...").

Use cases: Confirmation of assignment, acknowledgment of terms, compliance attestations.

Signature fields are most commonly used in Confirmation survey templates. While they can be added to any survey type, they are required for Confirmation surveys to capture the caregiver's formal commitment.

Rich Text Editor​

All question text fields support rich text formatting through an inline editor. The editor toolbar provides the following formatting options:

  • Bold, Italic, Underline -- Standard text formatting.
  • Bulleted List / Numbered List -- For presenting structured information within a question.
  • Hyperlinks -- Link to external resources, policy documents, or reference materials.
  • Headings -- H3 and H4 level headings for structuring longer question descriptions.
  • Block Quote -- For highlighting important information or policy language.

Use rich text formatting sparingly in questions. Bold and bullet points are effective for drawing attention to key details, but excessive formatting can make surveys harder to read on mobile devices.

Theme and Branding Configuration​

Each template can be customized with your organization's branding. Access theme settings by clicking the Theme tab in the template editor.

LogoUpload your organization's logo. Displayed at the top of the survey. Recommended size: 200 x 60 pixels.
Primary ColorThe main accent color used for buttons, progress bars, and highlights. Enter a hex code or use the color picker.
Background ColorThe background color of the survey page.
Font FamilyChoose from a set of web-safe fonts for the survey text.
Header TextOptional text displayed below the logo (e.g., organization name or event title).
Footer TextOptional text displayed at the bottom of every survey page (e.g., copyright notice, contact information).

Theme settings apply to the caregiver-facing survey only. They do not affect how the template appears in the admin editor.

Previewing a Template​

Click the Preview button in the top-right corner of the template editor to see the survey as caregivers will experience it. The preview opens in a new browser tab and renders the survey with all branding, question formatting, and signature fields.

Preview mode is interactive -- you can fill out answers and test the flow -- but responses entered in preview mode are not saved.

Editing an Existing Template​

  1. Navigate to Settings > Survey Templates.
  2. Click the template name to open it in the editor.
  3. Make your changes -- add, remove, or edit questions, update branding, or modify settings.
  4. Click Save to persist your changes.
caution

Editing a template does not retroactively update surveys that have already been sent in active campaigns. Changes only apply to new campaigns created from the template after the edit.

Duplicating a Template​

To create a new template based on an existing one:

  1. On the template list page, click the three-dot menu next to the template you want to duplicate.
  2. Select Duplicate.
  3. A copy of the template is created with the name "Copy of [Original Name]". Open it to rename and customize.

Deleting a Template​

  1. On the template list page, click the three-dot menu next to the template.
  2. Select Delete.
  3. Confirm the deletion in the dialog.
caution

You cannot delete a template that is currently in use by an active campaign. End or archive the associated campaigns first.
